HeliosRx LLC (“HeliosRx”) is a state-licensed 503A compounding pharmacy. This Notice of Privacy Practices (“Notice”) describes how we may use and disclose your protected health information (“PHI”), your rights regarding that information, and our legal duties under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).

Our Responsibilities

• Healthcare Operations: To operate, maintain, and improve our pharmacy services, conduct quality assessment, and comply with regulatory obligations.

• Public Health and Safety: To report adverse events, product concerns, or medication issues to the FDA or other authorities.

• Legal Requirements: To comply with subpoenas, court orders, or lawful requests from regulators or law enforcement.

• Business Associates: Vendors who handle PHI on our behalf must sign agreements to safeguard it.

• Other Uses: As permitted by HIPAA, including for health oversight, organ donation, workers’ compensation, and limited research purposes.

We will not use or disclose your PHI for marketing or sell your PHI without your written authorization.

Your Rights

You have the following rights regarding your PHI:

• Access: You may request to see or obtain a copy of your PHI.

• Amendment: You may request corrections to your PHI if you believe it is inaccurate.

• Restrictions: You may request limits on how we use or disclose your PHI.

• Confidential Communications: You may request that we communicate with you in a specific way (e.g., at a different address or phone number).

• Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.

• Paper Copy: You may request a paper copy of this Notice at any time.

Special Notes

• Text Messaging: Text messages are not secure. We limit health information included in texts and require opt-in consent.

• Children’s Information: We dispense prescriptions for pediatric patients only with a valid prescription; their PHI is protected under HIPAA.

• Veterinary Prescriptions: Information about animal patients is not PHI under HIPAA, but we treat it as confidential.
